CLIENTS
CLIENT 1
This client is a paediatric client on NDIS. Her mum has plan-managed NDIS funding. RN Junko attends to the client's day-to-day health needs. Junko's visit also helps her mum to be available to attend to the needs of her sibling and family.
CLIENT 2
This client has self-managing HCP level 4. RN Junko manages her diabetes foot ulcers twice a week as well as preventing further health deterioration. In this way, the client can maintain her independence at home.
CLIENT 3
Client 3 lives alone in the Hills. The client's son who lives in the city helps him to self-manage HCP level 4. RN Junko is responsible for the client's medication management and diabetes management. Junko visits him 6 times a week to keep the client's BSL within the acceptable level. Junko texts his son to report the client's health after each visit. The client's goal is to live independently at home.
CLIENT 4
This client with severe complex health issues has been on NDIS for 3 years and plan-managed NDIS funding. RN Junko visits the client twice a week to assess his physical and mental health and keeps the client from presenting himself to Emergency Department. RN Junko also keeps the client connected with his GP and allied health practitioners.
